A medical certificate is not a universal document requirement for every Costa Rica residency application. The DGME general requirements and the Pensionado and Rentista checklists reviewed for this guide do not prescribe a routine physical examination, blood tests and chest X-ray for every applicant.
Use the exact category and individual instructions. This conclusion about the lists reviewed is not a guarantee that medical evidence will never be requested in a particular case.
When medical evidence can be relevant
DGME’s Pensionado and Rentista dependent instructions identify a medical opinion for an adult child with a disability, together with judicial guardianship certification where applicable. The purpose is to establish that specific dependent circumstance, not to certify every applicant as generally healthy.
Other immigration procedures can have different requirements. A medical-treatment stay, a disability-related dependent application and an ordinary pension application should not be treated as one checklist.

Before paying for an examination
- Find the exact medical-document item in the official category checklist or your case notice.
- Ask what fact the opinion must establish and who may issue it.
- Confirm any required form, date, signature and professional identification.
- For a foreign document, check the applicable authentication and translation.
- Obtain a written quote for the requested service rather than ordering an invented test package.
Share the official request with the clinician. Do not ask for unsupported statements or assume a generic fitness certificate answers a specific disability question. Keep relevant records private and send them only through the appropriate channel.

Health insurance and treatment planning are separate
CCSS enrollment, private insurance underwriting and clinical treatment involve their own assessments. A residency filing does not automatically activate healthcare coverage. Conversely, an insurer’s medical questionnaire is not a DGME document checklist.
If you have an existing condition, plan continuity of care with your treating clinician and confirm coverage with the institution or insurer. Our healthcare planning guide provides questions about records, medicines and provider access.

Check the requirement for your file
Read DGME Regularización, including the general requirements and your category. If a notice requests additional evidence, clarify that notice before assuming the general list overrides it. Contact CRIE for help identifying the immigration document being requested.
